The Supplier Performance Maturity Model is designed to reflect the current state of the main aspects in client-supplier relationship. Created as a set of statements to be rated from a 1 to 5 scale, the Maturity Model explores elements such as: supplier selection, supplier contracting, design of service level agreements, KPIs used to measure and monitor supplier performance, supplier relationship management; supplier performance evaluation; risk and communication management with suppliers. Once the Maturity Model is filled, the entity can fall into one of the following maturity levels:

2. Emergent
3. Structured
4. Integrated
5. Optimized
